In my practice, existential assessment means evaluating the patient’s relationship to meaning, purpose, identity, and direction alongside their biological and psychological presentation. A patient whose depression persists despite adequate medication may not have a medication problem. They may have a structural misalignment between their life architecture and their deeper values. That misalignment generates symptoms that no prescription can resolve.

This is not philosophical speculation. It is clinical observation across hundreds of patients. The adults who present with treatment resistant symptoms are frequently carrying existential misalignment that no previous provider assessed.

The Alignment Framework Applied to Meaning and Direction

Alignment Psychiatry assesses where a person is fundamentally misaligned and what that misalignment is asking to be corrected.
