A patient whose SSRI produces partial response may not need a higher dose. They may need the thyroid dysfunction that is limiting the SSRI’s efficacy to be identified and addressed. A patient on three medications may not need a fourth. They may need the biological variables that necessitated the polypharmacy to be investigated.

Medication Tapering Is Not Subtraction

Many patients seek medication tapering. This is a legitimate clinical goal. It is also one of the most mismanaged processes in psychiatry.

Tapering is not subtraction.
